Los Angeles Rams wide receiver Puka Nacua was left bleeding from his eye after taking a helmet-to-helmet hit in the middle of the field during Sunday’s game against the Houston Texans and was evaluated under the NFL’s concussion protocol, the team said.

The hit forced Nacua from the play and he exited the field. The Rams initially listed him as questionable to return. He was seen on the sideline with a large bandage over his head and later re-entered the game in the second quarter. Broadcasters said he went into the medical tent before halftime, where medical staff appeared to attend to the dressing.

Television replays showed the contact as helmet-to-helmet with a Houston defender in the middle of the field. Following the play, Nacua was bleeding from the area near his eye and received immediate attention on the sideline. The Rams confirmed he was undergoing concussion checks; the team provided no further medical update during the game.

Nacua’s appearance on the sideline with a heavy bandage drew concern given his injury history. He struggled with injuries last season, missing time with a posterior cruciate ligament strain. Nacua played in Week 1 of the previous season but missed the next five weeks before returning in Week 7.

Rams medical personnel and independent medical staff follow NFL concussion-evaluation procedures when a player shows signs of a potential head injury. Those procedures include a sideline assessment and, if necessary, further evaluation in the locker room or a medical tent. The team’s initial designation of Nacua as questionable to return reflected the ongoing nature of that evaluation.

The incident raised attention because Nacua has been one of the Rams’ primary offensive playmakers since entering the league. Any head or facial injury that prompts a concussion protocol can affect a player’s availability and the team’s game plan. At the time of the broadcast update, there was no official word from the Rams on his status after halftime.

The game continued while medical staff attended to Nacua. The Rams and the NFL did not immediately release a postgame report detailing the results of the concussion evaluation or the nature of any facial injuries. Further updates on Nacua’s condition and whether he will be available for upcoming games are expected from the team’s medical staff or in official injury reports.
